Band 7 - Macmillan Breast Clinical Nurse Specialist
Role is a key position in the breast multidisciplinary team. To assess, plan, implement and evaluate expert nursing care and provide support to patients attending the breast unit with a suspected or diagnosed breast cancer.
Post holder will have continuing responsibility for the coordination and organisation of individual treatment programmes throughout the disease trajectory.
Job is 22.5 hours per week based at St Albans City Hospital.

Key Responsibilities
* Work autonomously to manage caseload of patients, whilst working as part of the multidisciplinary team; maintain effective communication with patients, carers and professionals to ensure seamless service delivery.
* Participate in Breast Clinics (results, follow up and metastatic) providing support and information for patients and relatives.
* Act as the coordinator of care for the patient, ensuring the patient moves through the pathway speedily and ensuring the relevant treatment pathway is upheld, including ensuring all relevant tests are carried out promptly and involving liaison with consultants within and outside the trust as appropriate.
* Assess new and ongoing metastatic patients in inpatient and outpatient settings; identify their psychological, physiological, emotional, spiritual and cultural needs and provide a pathway of nursing care for the patient.
* Provide specialist advice and psychological support.
* Develop communication pathways with key stakeholders across health and social care settings.
* Act as an expert resource and facilitator to educate and train other staff and students, helping to provide optimum nursing care for patients with breast cancer or related problems.
* Be involved in development of policies, quality and audit initiatives, and nursing research.
* Actively seek to improve patient care through service improvement either autonomously or in collaboration with the service improvement team.
