About the Role:
We are seeking an experienced and dynamic Clinical Lead to join our forward-thinking Complex Care and Mental Health provider. This is a field-based role with remote working, responsible for overseeing and supporting 7 community-based packages of care for clients with a range of needs, including:
🔵 Learning Disabilities (LD)
🔵Mental Health (MH) and challenging behaviours
🔵Complex physical health needs such as PEG, CPAP, epilepsy management, tracheostomy, and stoma care
You will play a pivotal role in ensuring the highest standards of safe, effective, and person-centred care are consistently delivered.
Key Responsibilities
🔵Conduct clinical audits, risk assessments, and care plan reviews.
🔵Write, review, and update individualised care plans.
🔵Provide competency sign-offs and deliver training to field staff.
🔵Offer clinical oversight and ongoing support for complex care interventions.
🔵Work closely with clients, families, and multidisciplinary teams to ensure holistic care.
🔵Respond to clinical incidents, escalating as appropriate.
🔵Ensure compliance with CQC and company policies at all times.
🔵Travel to client homes across Sussex (and occasionally Hampshire) as required.
Requirements:
✅ Registered Nurse (RGN/RMN/RNLD) with active NMC pin.
✅ Minimum 2 years’ post-graduate experience.
✅ Must be a car driver with own vehicle.
✅ Experience in community, complex care, or mental health settings.
✅ Ideally experienced with PEG, tracheostomy, and stoma care.
✅ Strong clinical governance knowledge.
✅ Excellent communication, leadership, and organisational skills.
✅ Ability to work autonomously and flexibly across a regional patch.
✅ Right to work in the UK (sponsorship not available).

There will be a Team Leader who reports into this role - who manages the support workers & carers on a day-to-day basis.
