Clinical Nurse Specialist, Community Team, Chelmsford
Farleigh Hospice is a high performing organisation providing specialist palliative and end-of-life care for people with life-limiting illnesses in the Mid Essex region. We are seeking highly motivated Clinical Nurse Specialists to join our Community team.
The role involves proactive case management of patients across a locality, working closely with other Clinical Nurse Specialists under the direction of the Team Manager. The team provides a responsive service offering specialist palliative care advice, symptom management, and holistic support to patients and families. You will also collaborate with primary care teams and other care providers to ensure the best quality of life for our patients.
This position emphasizes education and advice to health and social care professionals, promoting GSF and End of Life Care. The role requires working on a 7-day rota.
Candidate Requirements:
* NMC Level 1 Registration in Adult Nursing with substantial post-registration experience in palliative care or relevant healthcare setting
* Experience in community nursing and palliative care/oncology
* Understanding of end-of-life issues and community healthcare services
* Holistic assessment and symptom management skills
* Organised with confident decision-making abilities
* Effective time management and workload prioritisation
* Ability to inspire and educate others in palliative care
* Strong communication skills with patients, families, and healthcare staff
* Resilient, able to work under pressure
