Overview
Lincolnshire Community Health Service (LCHS) is looking for Registered Nurses to join the Boston Community Nursing Team. Community staff nurses work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will assess care needs, plan and evaluate nursing care using innovative evidence-based practice.
Responsibilities
* Assess care needs and develop care plans.
* Deliver nursing care in patients' homes, care homes and clinic settings.
* Provide wound management, continence care, palliative care, medication administration and long-term condition management.
* Collaborate with the multidisciplinary team – social workers, GPs and allied health professionals – to coordinate patient care.
* Maintain accurate clinical records and uphold patient safety standards.
Travel & Transport
Access to private transport is essential for travel to patients' homes and clinic sites, as public transport coverage is limited in rural Lincolnshire.
Working Hours & Shift Patterns
* Service hours: 8:00 am – 6:30 pm, seven days a week.
* Shift patterns: early (8:00 am – 4:00 pm), mid (9:00 am – 5:00 pm), late (10:30 am – 6:30 pm) and weekend shifts (8:00 am – 5:00 pm or 8:00 am – 6:30 pm).
