MDT Patient Tracker

Closing date: 12 April 2026

This is an opportunity for an enthusiastic and hardworking individual to join the Dermatology Department at UHBW.

The successful candidate will work alongside the Fast Track Team and Cancer MDT Coordinators, supporting the tracking of patients with suspected cancer from referral through to diagnosis and treatment, ensuring national cancer waiting time targets are met.

This is a fast‑paced role that involves significant learning. You will be expected to develop a strong understanding of cancer pathways, Trust systems and targets, and apply this knowledge accurately in your daily work. The ability to take ownership of patient pathways, follow tasks through from start to finish, and work independently using your own initiative is essential.

Strong organisational and communication skills are required, as the role involves regular liaison with clinical and administrative teams across the Trust. The successful candidate must be able to manage competing priorities, respond professionally to queries, and maintain accuracy under pressure.

Main duties of the job

* Responsible for ensuring patients on a 62 days pathway (62 day standard, screening and upgrade) reach the Faster Diagnosis Standard by day 28.
* Record accurate data on the Cancer Database (Somerset Cancer Registry).
* To liaise with the relevant outpatient department ensuring that patients’ first outpatient appointments or straight‑to‑test appointments are booked within the 28‑day target.
* Manage areas of the Cancer Patient Tracking Lists (PTL) ensuring all parts of pathways of care are delivered in a timely manner, e.g. arranging diagnostic or treatment procedures to be expedited when booked outside of the treatment date.
* Ensuring any capacity issues are escalated to the Deputy Performance & Operations Manager, Cancer Services via the weekly PTL checks. Where necessary issues should be escalated immediately without waiting until the meeting.
* To support Cancer MDT Coordinators, ensuring smooth administration of patients along their care pathway. To provide administrative assistance as required during periods of annual leave.
