Overview
The Community Health Manager will be responsible for all community health program design for Mt. Ascutney Hospital, development and implementation; community assessment activities; program reporting and improvement recommendations. This position will serve as the Blueprint Program Manager for the Windsor Health Service Area (HSA) in addition to being the facilitator for other organizational community-facing programs, including the Community Collaborative and the Community Health Implementation Plan (CHIP) workgroups. This position will be responsible for completing all applications, renewals, and reporting requirements for federal, state, and foundation-related grants across the community health department. Post procurement, they will be responsible for overseeing logistics and coordination — including efficient communication and reporting — assuring compliance with regulations, adhering to grantor requirements, communicating with both grantor and any subrecipients and/or contractors to assess the success of allocated programs and projects.
Responsibilities
* Design, development and implementation of community health programs for Mt. Ascutney Hospital; conduct community assessment activities; manage program reporting and provide improvement recommendations.
* Serve as the Blueprint Program Manager for the Windsor Health Service Area (HSA) and act as facilitator for other organizational community-facing programs, including the Community Collaborative and the CHIP workgroups.

* After procurement, oversee logistics and coordination, including ensuring efficient communication and reporting; assure compliance with regulations and grantor requirements; communicate with grantors, subrecipients, and/or contractors to assess the success of allocated programs and projects.
Salary
Salary range - minimum: $35.60, maximum: $55.18
