We need: Compassionate individuals with strong communication and organizational skills.
Our service focuses on promoting recovery for those diagnosed with severe and enduring mental ill health. We provide comprehensive multi-disciplinary services and work closely with local specialist mental health teams (e.g., Early Intervention Service, Home Treatment Teams, MIMHS), tertiary service providers, primary care, Kent Police, local statutory and non-statutory agencies, service users, and carers.
The post holder must hold a full, valid UK driving licence and have access to a car/vehicle for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
1. Focus on collaborative community mental health support, promoting engagement and independence.
2. Establish therapeutic relationships to address recovery goals, helping service users access resources and connect with the community.
3. Work closely with occupational therapists and assist nursing staff with depot/clozapine clinics, including well-being checks.
4. Visit people at their homes as required.
5. Complete Dialog plus assessments via telephone or in person.
6. Monitor and report on the mental state of service users.
7. Provide support and identify service users' needs.
8. Opportunity to run recovery activities with Occupational Therapists.
