Full Job Description
Muve Homecare is seeking a dedicated Care Coordinator to join theirComplex Care division in Birmingham. This role is pivotal in ensuringthe seamless delivery of personalized, high-quality care to clients intheir homes.
Key Responsibilities:
* Rota Management:
* Develop, manage, andupdate staff schedules to ensure 24/7 coverage.
* Promptlyaddress last-minute scheduling changes.
* Align rotas withclients' care plans and preferences.
* Recruitment andScreening:
* Actively recruit qualified caregivers.
* Screen candidates by reviewing CVs, conducting interviews,and verifying qualifications.
* Ensure compliance withregulatory standards and company policies during hiring.
* Onboarding New Staff:
* Facilitate theinduction process, ensuring new team members are well-trained.
* Provide ongoing support and mentorship during the probationperiod.
* System Management:
* Utilize caremanagement systems to track schedules, care plans, and compliancerecords.
* Maintain accurate documentation for clients andstaff.
* Identify opportunities to optimize processes andimplement improvements.
* Client and Staff Support:
* Serve as the main point of contact for clients, addressingqueries and concerns.
* Support staff, fostering a positive andcollaborative work environment.
* Conduct regular check-inswith clients and staff to ensure satisfaction and well-being.
Qualifications and Skills:
* Experience:
* Previous experience in care coordination, complex caremanagement, or a similar role.
* Skills:
* Strong organizational and time management abilities.
* Excellent communication and interpersonal skills.
* Proficiency in care management software and IT systems.
* Qualifications:
* Knowledge of CQC standardsand regulations (or equivalent).
* NVQ Level 3 in Health andSocial Care (preferred but not mandatory).
