To promote close and constructive relationships with individuals, their families and the local neighbourhoods. The post holder must be able to assess/support plan/review individuals receiving care and support from Adult Social Care with an emphasis on an Asset based approach. Liaise with colleagues in own and other council services and external agencies in order to gather information relevant to Assessment and Support Planning activities.
