Are you a passionate, clinically driven Pharmacist looking to make a real impact in community healthcare? An exciting opportunity has arisen for a Primary Care Network (PCN) Pharmacist to join a dynamic and supportive team across Cornwall.
Key Responsibilities:
* Conduct structured medication reviews and support medicines optimisation
* Manage repeat prescribing systems and ensure safe, effective prescribing
* Provide clinical advice to patients and the practice team
* Support chronic disease management and preventative care initiatives
* Contribute to audits, QOF, and IIF targets within the PCN
* Liaise with community pharmacy teams, secondary care, and wider services
About You:
* GPhC-registered Pharmacist
* Experience in a primary care, community, or hospital setting
* Independent Prescriber (desirable, or willingness to undertake training)
