The service triages and assesses all urgent and emergency referrals to identify the most appropriate service to meet individual needs when persons are experiencing a mental health crisis. This includes telephone triage, support and signposting as well as face‑to‑face full needs assessment, care planning and referral to relevant services.
The team currently consists of Community Mental Health Nurses, Psychologists, Consultant Psychiatrists and other associated medical staff. The service operates 24/7 over a varied shift pattern and remains focused on service‑user needs.
Responsibilities
* Triage all mental health referrals to the Crisis Team to determine priority for service response and appropriate clinical pathway.
* Carry out full assessments of needs, including comprehensive risk assessments for persons referred or self‑referred.
* Recommend and refer to the most appropriate team or service, utilising all available resources.
* Identify suitable treatment‑based interventions for all service users and ensure best practice is implemented.
* Provide telephone triage, support, signposting, face‑to‑face assessments and care planning.
