Experience Required (Essential)
You must have a minimum of 2 years’ experience working as a Care Coordinator within domiciliary and/or complex care. Hands-on care experience is a bonus. Please note that applicants who have only worked as Care Assistants or Support Workers, without Care Coordinator experience, will not be considered for this role.
About the Role
As a Care Coordinator at C4 Care, you’ll play a vital role in ensuring our clients receive safe, high-quality, person-centred care. You’ll work closely with service users, families and care teams to keep services running smoothly while adapting quickly to changing needs.
You’ll be planning and coordinating care visits, making sure rotas are effective and client needs are met. You’ll build strong relationships with service users, their families and your care teams, while leading, supporting and supervising care staff to deliver excellent care every day. The role also involves carrying out risk assessments, developing and reviewing care plans, responding promptly to emergencies or changes in care requirements, and ensuring all services remain compliant with CQC standards and company policies.
You’ll also be confident using scheduling systems and managing the administrative side of coordination. While this is not a hands-on care role, there may be extremely rare occasions where supporting with care is needed, so a willingness to step in if required is essential.
What We’re Looking For
We’re looking for someone with proven experience in a Care Coordinator or similar role within health and social care, with strong organisational and communication skills. You’ll have a caring, professional and person-centred approach, along with a good understanding of both domiciliary and complex care. You must hold a full UK driving licence, have access to your own vehicle, and be willing to travel to clients to carry out checks when required.
