Main duties of the job
We are now seeking a Care Coordinator to work as part of this multi-disciplinary team. Youll support the co-ordination of support to individuals and families across multi-disciplinary services within the Care Management Service. This involves processing referrals and ensuring patients get the right support from the multidisciplinary service at the right time. Youll be the central point of information and guide processes to improve working practices.

Job responsibilities
We are excited to share an opportunity to join the West Ipswich Care Management initiative which supports high users and predicted high users of acute non-elective beds. Care Management is a way of intensively supporting this group of patients with proactive care co-ordination and additional GP & Community Matron capacity.
We have started this service in the West Ipswich Integrated Neighbourhood Team area and our aims are:
* To improve the care of these complex patients
* Ease workload pressures across the system
* Reduce hospital admissions
