Hospital Social Worker - Hampshire
Location: Hampshire | Contract: 3 months rolling contract | Start Date: ASAP | Rate: £38ph
About the Role
As part of the Hospital Discharge Team, you will:
* Manage a caseload of patients requiring safe and timely discharge from hospital settings
* Assess individual needs and develop, review, and implement effective discharge plans
* Complete relevant assessments, documentation, and reports to support decision‑making
* Work closely with healthcare professionals, families, and multi‑agency partners to coordinate care
* Ensure individuals experience a smooth transition from hospital to home or appropriate care settings, promoting their safety, independence, and wellbeing
Requirements
* Qualified Social Worker (SWE registered)
* Previous Hospital experience
* Strong safeguarding knowledge
* Ability to work effectively in a fast‑paced environment
* Eligible to work in the UK
* Drive and have access to a car
* Post‑qualified experience
* Degree level or equivalent in Social Work
