Job Summary
The Channel Community Care Coordinator is responsible for supporting individuals within the community by delivering high-quality, person-centred care services. The role involves coordinating care plans, liaising with families and external professionals, and ensuring that all services are delivered in line with regulatory standards and organisational values.
Key Responsibilities
* Deliver and coordinate person-centred care that promotes independence, dignity, and wellbeing.
* Act as a key point of contact for service users, families, and relevant stakeholders.
* Develop, implement, and regularly review individual care plans tailored to service users’ needs.
* Ensure compliance with safeguarding policies, health and safety regulations, and care standards.
* Monitor and assess the quality of care provided, taking corrective action where necessary.
* Maintain accurate and up-to-date records in line with data protection requirements.
* Support service users in accessing community resources and activities.
* Work collaboratively with healthcare professionals, social workers, and partner organisations.
* Provide guidance and support to care staff, promoting best practices in community care delivery.
* Participate in training and continuous professional development.
Skills and Experience
* Proven experience in community care, social care, or a related field.
* Strong understanding of person-centred care principles and safeguarding procedures.
* Excellent communication and interpersonal skills.
* Ability to manage multiple priorities and work independently.
* Strong organisational and problem-solving skills.
* Proficiency in record-keeping and basic IT systems.
Qualifications
* Relevant qualification in Health and Social Care (e.g., Level 3 Diploma or equivalent).
* Additional certifications in safeguarding or care management are desirable.
