Social Worker Salary £39,152 - £44,075 per annum Type of vacancy Permanent Work pattern Full Time Hours per week 37 Work Base County Hall, West Bridgford, NG2 7QP Working Arrangements Hybrid Job Description / Person Specification Competency Framework Number of posts 1 Interview date To be confirmaed Closing Date 19 May 2026 23:55 Nottinghamshire County Council is developing an improve all-age Mental Health offer, alongside its existing work for people with Learning Disability and Autism which will involve strengthened multi-disciplinary working with a variety of partners. This post will sit in the Mental Health Discharge Team and the work will primarily be with those individuals on acute psychiatric wards and involve assessment and discharge destination planning. This will promote best practice, manage demand and respond to legislative change, with a focus on reablement. We want the people we serve to live good, ordinary lives in the place they call home with the people and things that they love, in communities where they look out for one another, doing what matters to them. If people need some support, we start by understanding what a good life looks like for them, and how we can we work together to achieve it. We are looking for dynamic staff with relevant knowledge, skills and expertise in Mental Health, to facilitate hospital discharge. You'll be responsible for: building trusted relationships having conversations with people based on what matters most to them making sure people's wishes, feelings and beliefs are central to decision making supporting people to live as independently as possible, for as long as possible connecting people to wider networks of opportunity and support increasing people's hope, self-esteem and creative potential. The role will ensure the delivery of high-quality, person-centred support whilst playing a pivotal role, working with partner organisations, to shape and improve Mental Health, Learning Disability and Autism services for local people. Essential requirements include: recognising that people are the experts in their lives communicating respectfully with and about people thinking and acting creatively challenging circumstances and environments that marginalise, exclude or oppress critical reflection and a desire to keep learning This is an exciting time for Adult Social Care as we implement new operational arrangements, ensuring that our services are fit for the future, with a focus on equity and individual outcomes to develop independence, alongside place based and partnership working with both internal and external partners. You will also contribute to the transformation of wider Adult Social Care services in line with the Council's strategic priorities and embed our Owning and Driving Performance Programme.
