Health Care Assistant (Fixed Term Until June 2026)
Join HCRG Care Group as a Health Care Assistant for a fixed term until June 2026. You will work closely with our Community Nursing Teams, supporting senior staff to provide care to patients in their own homes for a range of conditions including diabetes, wound care, and end‑of‑life care.
Job Introduction
Our community nursing team empowers staff to work autonomously and flexibly. Your role includes using state‑of‑the‑art tablets to check in from home, maintain notes on the go, and avoid being trapped in an office base, enabling you to truly enjoy working in a community setting.
Key Responsibilities
* Assist in providing nursing care as delegated by the Team Leader within patients’ homes.
* Maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
* Provide clerical and administrative support to the team as delegated by the Senior Sister/Team Sister.
* Deliver care in accordance with organisation policies, procedures and guidelines, according to competencies.
* Report any changes in condition of the patient to the Team Leader or, in their absence, the designated nurse in charge of the caseload.
Qualifications & Experience
* NVQ Level 2/3 or equivalent qualification/experience.
* Strong communication and organisational skills.
* Flexible approach, delivering positive experiences to patients throughout their healthcare journey.
* Valid driving licence – able to travel to service users’ homes in West Lancashire.
* Comfortable using IT systems – working knowledge of Microsoft Office (Outlook, Word, Excel).
* Encourages nursing students seeking hands‑on experience in community care.
