Overview

Specialty Doctor in Psychiatry to join the Crisis Resolution and Home Treatment Team (CRHTT) at Central and North West London NHS Foundation Trust (CNWL), based at Milton Keynes Hospital Campus. This is a rewarding opportunity to contribute to a dynamic, multidisciplinary service delivering urgent mental health care in the community. Closing date: 05 October 2025.

We welcome applicants with previous experience in Psychiatry who are highly motivated, compassionate, and committed to delivering patient-centred care. Strong communication, organisational skills and the ability to work both independently and collaboratively within a team are essential.


Main duties of the job

You will provide high-quality psychiatric assessments, consultations and treatment to service users in acute mental health crisis. Working alongside a Consultant Psychiatrist and Trainee Doctors, you will play a key role in clinical care and decision-making within the team, and will offer strategic leadership alongside the CRHTT Manager and Service Manager.

The post-holder will have RC responsibility for all patients under the care of the team and will work Monday to Friday, 9am to 5pm, although flexible working hours can be considered.


About us

The Trust has a long history of close working with Milton Keynes Council and Milton Keynes University Hospital to create joint services. Intermediate care, community equipment and learning disability services are integrated across health and social care and are provided through pooled budgets and integrated teams. We are part of the Bedfordshire, Luton and Milton Keynes (BLMK) Integrated Care Board (ICB).

The CRHTT is a multi-disciplinary team who provide a service to people experiencing an acute psychiatric crisis. The team offers a less restrictive alternative to hospital admission by delivering intensive community-based interventions. Where hospital admission does occur, the CRHTT assists in shortening the inpatient stay by facilitating early discharge and support back to the community. The team has five main functions: crisis rapid response, gatekeeping, supporting admission, home treatment planning and acute care with early discharge.

Job responsibilities

Clinical Duties

Assessment, diagnosis, formulation of risks and development of a management plan for new referrals, re-referrals and patients under the care of the team. Referrals can come from any service including GPs, A&E psychiatric liaison, inpatient wards, community mental health services. Some assessments and reviews take place at the team base, others in patients’ homes or ward settings. Relevant travel time will be factored in when arranging home visits.

Recording Clinical Coding (Diagnosis) on patient records.

Requesting relevant investigations such as blood tests, ECGs, MRI brain scans where appropriate and ensuring relevant baseline investigations prior to starting psychopharmacological interventions.

Discharging medical prescribing functions as needed in the team, which may involve: providing FP10s, writing medication charts, initiating, reviewing and advising on psychopharmacological interventions as appropriate; liaising with pharmacists who carry out weekly screening of drug charts; reconciling medication for patients seen through GP summary care records and progress notes.

Identifying need for referrals to other services such as referrals for psychological interventions.

Reviewing treatment plans and risk assessments of patients in the service, providing support and advice to MDT colleagues and liaising with other professionals as required.

Chairing MDT clinical meetings during consultants’ leave and reviewing minutes of those meetings.

Ensuring appropriate handover takes place during involvement in the assessment of patients referred from the ward.

Ensuring comprehensive documentation on patient records following patient assessments, telephone calls and other liaison with professionals.

Supporting trainee doctors as required, for example during the consultants’ leave.

Participating in and contributing to MDT teaching sessions, reflective practice sessions and any Serious Incident Learning sessions.

General Duties

Undertake administrative duties associated with the care of patients.

Participate in service and business planning activity for the service and, as appropriate, for the whole mental health service.

Participate in annual appraisal.

Attend and participate in the academic programme of the Trust, including lectures and seminars as part of the internal CPD programme.

Maintain professional registration with the General Medical Council and abide by professional codes of conduct.

Participate in annual job plan review with Clinical Supervisor.

Work with Consultants, local managers and professional colleagues to ensure the efficient running of services.


Person Specification


Qualifications and Eligibility

* Primary Medical Degree
* Full GMC Registration
* GMC Licence to Practise
* Section 12 MHA Approved
* An additional postgraduate qualification of relevance (own specialty)


Clinical Skills, Knowledge & Experience

* Experience as a fixed-term trainee or as a Specialty Doctor on a part-time or flexible basis in general adult psychiatry.
* Experience in working in a community mental health team/primary care mental health.
* Evidence of audit/quality improvement experience.


Personal Skills & Qualities

* Excellent communication and empathic skills, including strong oral and written English.
* Timely and effective verbal and written communication with patients, carers, colleagues and the MDT.
* Demonstrable leadership and team-working abilities.
* Sound organisational skills and the ability to work on own initiative with evidence-based decision-making.
