About the role:
1. Act as a key liaison between prison in-reach teams and community drug/alcohol services to support continuity of care, shared case management, and coordinated risk reduction planning.
2. Carry out prison-based screenings, initial assessments, and risk assessments, developing recovery plans in partnership with service users prior to release.
3. Provide harm reduction education, overdose prevention, and relapse management planning with service users in custody and following release into the community.
4. Deliver and promote Naloxone provision pre- and post-release, and train service users and peers in its use where appropriate.
5. Offer “through-the-gate” support including gate pickups where appropriate and facilitate access to community recovery services, housing, and health appointments upon release.
6. Conduct home visits, hostel visits, and community outreach as needed to maintain engagement, particularly with high-risk individuals and those at risk of disengagement.
7. Work collaboratively with police, probation, mental health, housing, and social care partners to address the complex needs of high-risk service users and promote recovery and rehabilitation.
About you:
8. Empathy and Understanding: A strong grasp of substance misuse issues and experience working in similar fields.

16. Approved for Restricted Institutional Access: The successful candidate will need to complete an enhanced DBS check and obtain HMPPS security clearance
