Westcroft Health Centre is looking for a motivated and patient-focused Pharmacist to join our multidisciplinary team. This is an excellent opportunity to play a key role in supporting safe, effective prescribing and improving patient outcomes within a busy GP practice.
You will work closely with GPs, nurses, and administrative staff to provide high-quality pharmaceutical care, with opportunities to develop your clinical skills and take on extended responsibilities. You will also be responsible for offering support to general practice staff in relation to prescription and medication queries as well as conducting medicines reconciliation on transfer of care.
Main duties of the job
* Carry out medication reviews and provide advice to patients on their medicines
* Support safe, cost-effective prescribing in line with current guidelines
* Respond to medication queries from patients and the wider clinical team
* Review repeat prescriptions and manage medication requests
* Work with GPs and nurses to optimise treatment plans
* Monitor and improve prescribing practices across the practice
* Support patients with long-term conditions to get the most from their medicines
* Contribute to audits and quality improvement work
* Provide support with medicines reconciliation following hospital discharge

Person Specification
Experience
* Experience working in primary care, community pharmacy, or hospital pharmacy.
* Strong clinical knowledge and understanding of prescribing practices.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a multidisciplinary team.
* Good organisational skills and attention to detail.
* Evidence of continuing professional development.
* Good critical appraisal skills.
* Experience of patient consultation.
* Experience of working with GPs.
* Good understanding of primary care structure and general practice.
* Good clinical knowledge.
* Able to apply clinical knowledge, appropriately, in a variety of practice settings or patient groups.
* Right to work in the UK. Please note that there is no sponsorship available for this role.
* Covid vaccination training.
* Advanced assessment and history taking skills or be willing to enrol in a current training pathway to develop these.
* Post-registration experience in primary, secondary or community pharmacy.
* Experience of clinical pharmacy.
* Experience using GP clinical systems (e.g. SystmOne) is desirable.
* Non-medical prescriber registration.
Qualifications
* Qualified Pharmacist with current registration with the General Pharmaceutical Council.
