What is the job? An Advanced Clinical Practitioner (ACP) working for Montu will practise within their advanced clinical competencies as part of a multi-disciplinary team (MDT), delivering high-quality patient-centred care in relation to Cannabis-Based Products for Medicinal use (CBPM).The ANP will provide comprehensive remote clinical assessments, structured consultations, medication reviews, and ongoing monitoring of patients prescribed CBPM. The role includes analysing complex clinical information to assess clinical need, independent prescribing, medicines optimisation, ensuring safe transfer of care, and contributing to systems that support safe and effective prescribing practices. This is a pivotal role focused on improving quality of care, patient safety, and operational efficiency. It requires strong clinical leadership, motivation, and a passion for delivering excellent, evidence-based care. What will you be doing? Conduct initial clinical consultations using video consultation to assess patients’ medical history, current treatment, and eligibility for CBPM treatment (training provided). Undertake remote follow-up consultations for patients prescribed CBPM, providing ongoing monitoring, review, and optimisation of treatment plans. Recognise and respond to changing clinical needs, identifying deterioration and taking appropriate clinical action and escalation where required. Independently prescribe CBPM in line with national guidelines, clinical governance frameworks, and organisational policies. Work collaboratively with consultants and other clinicians within the MDT to ensure safe, evidence-based prescribing and monitoring. Support safe medicines management, including reviewing treatment efficacy, side effects, interactions, and risk factors. Manage repeat prescribing reauthorisation processes by reviewing patient requests and identifying medicines requiring review within agreed SLAs. Liaise closely with partner pharmacies to ensure accurate dispensing and timely delivery of medications. Maintain accurate, contemporaneous electronic patient health records following consultations and prescribing decisions. Act as a Level 3 Safeguarding practitioner, identifying, assessing, and escalating safeguarding concerns appropriately. Implement prescribing changes in response to MHRA alerts, product withdrawals, and other relevant national or local guidance. Participate in clinical audits to ensure safety, compliance, and continuous quality improvement. Work alongside governance and operational teams to ensure compliance with CQC standards where medicines management is involved. Contribute to service efficiency and sustainability through monitoring, audit, and quality improvement initiatives. Attend clinical supervision sessions to support reflective practice and professional development. Maintain all mandatory training and remain compliant with company policies, SOPs, and professional regulatory standards. Attend work-related meetings as required. Work flexibly on a shift basis where required. Collaborate with the Clinical Leads and Head of Clinical Services and wider team to ensure KPIs and service targets are achieved. Work closely with the operations team to ensure timely and effective patient care delivery. What do you need? Professional regulated core qualification with the NMC/HCPC Qualified Advanced Nurse Practitioner (Master’s level or equivalent advanced practice qualification). Independent Prescriber qualification (V300). Minimum of 4 years post-qualification experience, with significant experience in advanced clinical assessment and prescribing. Strong clinical assessment, diagnostic, and decision-making skills. Evidence of continued professional development (CPD); completion of a CPD-accredited medical cannabis educational programme is desirable (training can be provided). High level of professional accountability and understanding of scope of practice. Self-motivated with excellent attention to detail. Professional, ethical, and committed to delivering high standards of patient care. Excellent communication skills and confidence conducting remote consultations. Strong digital literacy with working knowledge of electronic health records and Microsoft Office applications (Word, Excel, PowerPoint). Ability to work autonomously while contributing effectively within a multidisciplinary team environment.
