The job itself

The Haltemprice Community Mental Health Team (CMHT) is a multidisciplinary team formed by staff from both East Riding Council and Humber Teaching Foundation Trust. An exciting opportunity to join one of East Riding's Community Mental Health Teams as a Social Worker has arisen, due to the post holder being on maternity leave.

This post is a social work role forming part of our integrated partnership arrangements with Humber Foundation Trust, providing mental health services in East Riding of Yorkshire. The Haltemprice Adult CMHT is a joint Local Authority and NHS team which covers a wide geographical area, providing secondary mental health services to the people living in the Haltemprice area. As a social worker in this team, you would be part of the social care team, ensuring that the Local Authority meets their statutory duties and people get a service from Adult Social Care.

The post provides appropriate Social Work advice and support in a multi-disciplinary team focusing on providing an effective range of interventions for an identified group of adults needing mental health support. The post holder would be responsible for promoting independence and designing appropriate personalised support, to build community resilience.

The post provides an effective range of specified social work interventions for adults, their families and carers. The post looks to build and support links to wider adult social care services, and NHS services to ensure support provided is holistic and person centred. This will include supporting people to remain well and independent and designing appropriate personalized support planning, aimed at intervention to maximize recovery and independence. The role will also ensure that the statutory duty of the Local Authority to prevent, reduce, and delay the development of needs for care and support by taking the necessary steps. The role will also include completing care and support assessments, support planning, and completing section 42 safeguarding enquiries under The Care Act. It will also include implementing any commissioned care as needed.

The post will ensure that all adult legislative requirements are adhered to and will provide the legislative advice to other professionals in the Multi Disciplinary team, and in other services.

The role requires progressive development and demonstration of the following:

Grade 6B: To provide and support the delivery of high standard social work practice within a multi-disciplinary team in mental health services. To manage a caseload of people experiencing poor mental health, signposting, assessing the need for services, providing and identifying effective support and intervention and where appropriate with other health staff in the Community Mental Health Team, co-producing plans with individuals that will meet their needs, and reviewing plans to ensure they are effective. To provide and support social work practice with increasing decision-making autonomy in situations of increasing complexity, risk, uncertainty and challenge. Being able to manage complex case work and offer expert opinion within the organisation and to others; offer expert support to case discussion and more formal meetings when appropriate and produce high quality assessments as required. Having a good understanding of community and voluntary services in order to maximise independence and empower individuals, which focuses on The Care Act s function of prevent, reduce and delay. Contributing to high quality social care service delivery in mental health.

Grade 7A: All the above, plus: To provide and support expert and effective social work practice for adults who are experiencing mental health difficulties. Being able to work as an autonomous professional, demonstrating expert and effective practice in complex situations, assessing and managing higher levels of risk, striking a balance between support and control, liaising with a wide range of professionals, including more senior levels. Being able to model good practice to others and supervise staff. To promote the links and inter-dependencies with wider agencies working in the social Care and health arena for the promotion of well being and independence of the individual.
