Symphony Healthcare Services has an opportunity for an experienced Acute Practitioner to join our passionate, multidisciplinary teams across the SSW Primary Care Network Practices.
The post-holder will be expected to work flexibly across all practices within the South Somerset West Primary Care Network, including:
* Martock Surgery
* South Petherton Surgery
* Hamdon Medical Centre
* Crewkerne Medical Centre
The hours available are 9.5 hours per week. Working pattern to be discussed at interview.
The annual salary for this post is £47,565 per annum (pro rata for part-time workers) with options to join either an NHS or NEST pension, among other benefits.
Main duties of the job
The post-holder will provide care for acutely presenting patients, including history taking, clinical assessment, diagnosis, treatment, and follow-up coordination, within their professional boundaries. They will demonstrate safe, evidence-based clinical decision-making for patients in the same-day urgent care setting, following published pathways, policies, and protocols.
Professional body requirements for practice must be met to work at this level.

Job responsibilities
You will be an experienced nurse/practitioner providing care for presenting patients, including history taking, assessment, diagnosis, treatment, and evaluation, within your professional boundaries.
You will demonstrate safe, advanced clinical decision-making and expert care, including diagnostic skills, to meet patient needs within the general practice setting.
You will also apply critical thinking in decision-making and collaborate with the practice team to support policy and procedure adherence. NMC requirements for advanced practice must be met.

Qualifications
* GCSE or equivalent in English and Mathematics
* A Level qualifications or equivalent
* Evidence of ongoing professional development
Knowledge and Skills
* Excellent interpersonal and communication skills
* Understanding of primary care environment
* Knowledge of NHS GP Contract
* Strong analytical skills, including data analysis
* Time management and workload prioritization
* Commitment to quality and safety
* Ability to collaborate effectively with partners
* Understanding of the South Somerset Symphony Programme and new care models
* Experience with EMIS searches and protocols
* Ability to multitask and handle unpredictable work patterns
* Willingness to work outside normal hours when required
Experience
* Qualified paramedic or specialist paramedic registered with HCPC
* Minimum of 5 years paramedic experience
* Evidence of CPD activities
* Ability to assess and manage patient risk safely
* Healthcare-related diploma or degree
