Are you looking for an exciting change and for a secondment to a new service? We are looking to recruit a dynamic and innovative Band 6 nurse to join the Integrated Community Response Service- a multidisciplinary team made up of GPs, an Advanced Clinical Practitioner, occupational therapists, a social worker, physiotherapists, nurses, a community matron, and support workers. The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. We also have short term funding for a Hospital at Home pathway.
The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.

We are currently seeking a nurse to work across all 3 service pathways, with a broad range of previous experience across acute and community settings with some previous experience working in accident and emergency and in treating acutely unwell clients in the community.
Main duties of the job
The successful candidate will have excellent assessment and sound clinical skills and be able to treat clients who present with varying medical diagnoses.
You must be flexible, organised, have excellent time management skills to manage a case load effectively as the team priorities change frequently throughout the day.
The postholder will have overall responsibility for day-day management of their clinical caseload and ensure all care is delivered according to Trust policies and procedures and professional guidelines
As the service is community based, you will ideally need to be a car driver or be able to access the community via bike or public transport and will be required to work a shift pattern across weekdays and weekends on a rota basis.

Essential criteria
1. Current NMC Registration Registered Nurse (RN)
2. Evidence of professional up-dating
3. Academic ability to study at degree level
4. Mentor Qualification
Desirable criteria
5. Nurse/Prescriber/Non-Medical Prescriber
6. District nurse qualification, certificate, DiP, Degree
7. Relevant post registration course and experience e.g. Palliative Care, Diabetes and Tissue Viability
Experience

8. 3 years post registration experience
9. Has experience of team working that will assist the individual to manage an effective team
10. Experience of ensuring practice is based on sound evidence and clinical audit
