Outreach and Digital Pathway Nurse (Band 6)

The post holder will have a pivotal responsibility in identifying, supporting, and facilitating the early discharge of patients through the utilisation of remote monitoring technology. Collaborate with multidisciplinary teams across both acute and community settings to determine patient suitability for early supported discharge, ensuring safe, effective, and person‑centred care delivery. This role is integral to admission avoidance, minimising hospital length of stay, and enhancing patient outcomes via technology‑enabled care.
* Participate in daily huddles to identify patients suitable for early supported discharge using remote monitoring technology.
* Identify and assess patients suitable for early supported discharge and remote monitoring.
* Provide day‑to‑day coordination of outreach activity, ensuring effective communication between acute, community and Digital Care Hub team.
* Coordinate safe onboarding of patients, ensuring understanding and engagement.
* Support safe transition of patients from hospital to community setting with appropriate monitoring plans in place.
* Provide patient and carer education on remote monitoring equipment and digital engagement.
* Act as a clinical liaison between the Digital Care Hub, community teams and acute services to ensure seamless communication and continuity of care.
* Lead on the development and implementation of new digital pathways that align with Digital Care Hub services.
* Develop good Trust‑wide working relationships and communicate effectively with nursing colleagues and members of multi‑disciplinary team.

Qualifications
* Understanding of hospital discharge processes, patient flow and integrated care pathways.
* Able to perform holistic assessment of patients to ascertain suitability for early supported discharge and remote monitoring.
* Able to design and implement new digital pathways.
* Coordinate daily workload and outreach priorities.
* Demonstrates professional accountability and adheres to the NMC/HCPC Code of Professional Conduct.
* Participates in the multi‑professional review of patients who have been referred to the DCH service, including those recently discharged from the hospital.
* Maintain accurate electronic patient records in line with the NMC Record Keeping Guidelines, ANHSFT Health Records policy and the Records Management Code of Practice 2020.
* Offer help and advice to patients, families and carers responding to their individual needs in a caring and supportive manner.
* Adhere to Airedale NHS Foundation Trusts policies and procedures relating to the Clinical Governance Framework.
