Overview
We are seeking a dedicated and organised Care Coordinator to join our team. The ideal candidate will play a crucial role in ensuring that clients receive the highest level of care and support. The Care Coordinator will be responsible for liaising between clients, families, and healthcare professionals to facilitate seamless care delivery.
Responsibilities
* Coordinate and manage client care plans, ensuring all necessary services are provided in a timely manner.
* Serve as the primary point of contact for clients and their families, addressing any questions or concerns regarding care services.
* Maintain accurate records of client interactions, care plans, and progress notes in compliance with organisational policies.
* Collaborate with healthcare providers to ensure comprehensive care is delivered to clients.
* Schedule appointments and follow-up visits, ensuring efficient use of resources and time management.
* Assist in the training and orientation of new staff members as required.
* Participate in regular team meetings to discuss client progress and share best practices.
Experience
* Previous office experience is essential, ideally within a healthcare or social services environment.
* Strong organisational skills with the ability to prioritise tasks effectively.
* Excellent verbal and written communication skills, with a focus on building rapport with clients and their families.
* Proficiency in using office software and electronic health record systems is advantageous.
