We are seeking to recruit a Band 6 Social Worker to join our fantastic multi-disciplinary teams within the hospital to provide assessment, risk management, care and treatment to aid recovery and further the patient journey. We are looking for an enthusiastic, compassionate, experienced and highly motivated skilled professional social worker to join our teams, in a challenging but rewarding role, to make a difference to the lives of our patients.
Responsibilities
* Provide assessment, risk assessment and care planning for patients detained in a secure healthcare setting, working within a defined caseload.
* Work within consultant-led multidisciplinary teams to identify and manage risk and ensure throughcare support is available; directly support both patients and their families.
* Take a lead role for safeguarding adults within the service and for safeguarding children who come into contact with the service.
* Provide social work reports to Mental Health Tribunals and to Care Programme Approach meetings on a regular basis.
* Contribute to initial assessments and coordinate with internal and external services as part of risk management and care planning.
Qualifications and Skills
* Bands 6 Social Worker or equivalent; registration with the appropriate regulatory body.
* Excellent interpersonal skills and the ability to work effectively as part of a multi-disciplinary team.
* Commitment to providing the highest standards of care; ability to promote and maintain high standards through communication, motivation, supervision and role modelling.
