We are looking for an experienced and motivated Qualified Social Worker (QSW) to join Rochdale Adult Care's Hospital Discharge Team. Based in Middleton, you will cover all hospital discharges across various sites, including out-of-borough hospitals. You'll be working as part of our Home First Hospital Discharge Hub, supporting Rochdale residents to return home or move to the most suitable care setting safely and promptly. This is a crucial role where you'll help people regain independence and ensure the right support is in place at the right time.
