We are looking for a new member of the dementia crisis team, providing care and therapeutic intervention to patients in a range of community settings under the guidance of professionally qualified members of the team. The post holder should have a passion for assisting disabled and/or vulnerable adults to live at home with dignity and independence while supporting them by providing rehabilitative care. The post holder would need to undertake a range of delegated clinical patient care duties in a range of settings, to maintain patient records and to assist in the ongoing development and evaluation of the service.
Responsibilities
* Provide care and therapeutic intervention to patients in community settings under professional guidance.
* Undertake delegated clinical patient care duties and maintain patient records.
* Support the ongoing development and evaluation of the dementia crisis service.
* Contribute to a holistic, patient-centered approach to enable home-based living with dignity and independence.

* Caring and compassionate approach with a passion for working with people with dementia and disabled/vulnerable adults.
* Ability to work as part of a team and to engage in service development and improvement.
