We are looking for an enthusiastic and caring Care Coordinator to join our Care team. Reporting to Registered Manager.
You will be working for the award-winning Care Home in the beautiful village of Otterton, near Budleigh Salterton - The Old Vicarage Care Home, rated 'Outstanding for Caring' by the CQC.
Key objectives of the role
* To lead by example and demonstrate in your actions and by the teaching, mentoring and supervision of others, the ethos and philosophy of The Old Vicarage.
* To support the Head of Care and the Registered Manager in achieving the Aims of the Home as set out in the Staff Handbook and to assist in resolving various matters as they arise.
* To be responsible for meeting quality assurance standards for delivery of care to the residents of The Old Vicarage. To ensure the co-ordination of care through the audit, organisation and delegation of tasks throughout the care team.
Key responsibilities
* To be responsible for the training, mentoring and supervision of members of the care team in all aspects of their work within the Home.
* To support the Registered Manager in maintaining compliance with CQC regulations while maintaining the ambience, philosophy and ethos of the home.
* Assisting the Registered Manager with Health and Safety responsibilities of the home.
* Working alongside the Registered Manager to ensure that resident's care needs are being met.
* Timely referral of residents to appropriate care and social services and liaising with healthcare professionals when they visit the home.
* Liaising with relatives, friends and next of kin to keep them informed of resident welfare issues.
* Undertaking the morning and lunchtime medication round. Responsible for booking in and returning medication to the pharmacy.
* Responsibility for making preparations for new and short stay residents and ensuring that new and short stay residents are made welcome and oriented to the home.
* Responsibility for maintaining up to date information in resident care plans and profiles.
* To provide on call responsibility as per the weekly rota.
* As Care Coordinator, responsible for the empowerment of team Leaders and Care Staff to resolve resident issues and problem solve alongside senior staff as required.
* To prevent and control the spread of infection and to follow the home's infection prevention and control policies.
Knowledge and Qualifications
* Senior Care Assistant/ Team lead experience is preferred.
* NVQ3 is preferred but not mandatory
