We have a population of up to 10,000 patients and we lead the way for Primary Care in Huddersfield. Our Practice's QOF figures are consistently high and we provide scope for our Clinicians to develop special interests in Primary Care.
Main duties of the job
The successful candidate will be responsible for managing patients with long term conditions as part of a large, experienced clinical team including GPs, ACPs, Practice Nurses, Pharmacists and Health Care Assistants. We will welcome part time applicants to be considered for this opportunity.
Salary will be negotiable, depending on experience.

We are asupportive training practice with 5 GP partners, Salaried GP, ACP and 2 GPRegistrars. We have excellent supporting administrative and clinical teamsincluding Practice Nurses, Healthcare Assistants and Pharmacyteam. We have approximately 9,700 patients and operate from purpose builtpremises. We are a member of the Tolson Care Partnership Network. We use TPP SystmOne and are high QOF achievers with aGood CQC rating.

We run many clinics for themanagement of chronic diseases such as asthma and diabetes and offer a widevariety of other medical services including antenatal and postnatal care, minorsurgery, childhood vaccinations, sexual health services and well-personcheck-ups.
Job responsibilities
Large supportive in-house multi-disciplinary team
The role will include proactive support for some or all of the areas:-
Diabetes and other Chronic Disease management
Health Promotion and screening, Travel Health, Cervical cytology, Sexual Health, Baby, Childhood and Adult vaccinations

Person Specification
Qualifications
* Qualified Nurse.
* Registered with NMC.
* General Practice Nursing experience.
* Cervical screening.
* Long term condition management, ie. Diabetes, Respiratory, Cardio-Vascular.
* Complex wound care.
* Immunisations and vaccinations.
* General Practice Nursing responsibilities.
