Overview
Adult Social Care Practitioner - Health Interface Service (Adults) at Somerset Council. Hospital-based social work team supporting Somerset residents to return to their homes when possible. Aim to enable safe discharge and support independence through collaborative, person-centred practice.
What you will be doing
* Admission avoidance and prevention ensuring people are safe.
* Person-centred discharge planning using a strengths-based approach.
* Multi-disciplinary working with Occupational Therapists, Physiotherapists and other medical professionals.
* Advocating for people and for carers to ensure people’s voices are heard.
* Encouraging independence and helping people build confidence to use available support.
* Identifying concerns and goals with people and families and planning required support.
* Developing knowledge of the local area and working with local agencies and village agents.
* Providing creative, out-of-the-box solutions.
* Progressing to a seven-day service (currently six) and contributing to a transfer of care hub initiative.
* Making evidence-based decisions in a fast-paced environment.
* Contributing to the development of Somerset’s new Adult Social Care service.
What kind of experience or qualifications you need
* Hold or be willing to work towards an NVQ level 3 in Health and Social Care or equivalent (health and social care related).
* Experience in a caring or supportive role.
* Willingness to explore innovative approaches to better support people.
* Passion for advocating for others and enabling independence.
* Innovative, creative and solution-focused mindset.

Additional information
* Location: Yeovil District Hospital (travel to hospital locations required).
* This role is part of the Grow Your Own scheme — after 12 months in post, we can consider sponsorship to become a fully qualified social worker with tailored development support.
* Grade 12 post with starting salary £28,598 rising to £32,061 per annum. Appointments normally made at the bottom of the salary scale.
* DBS disclosure check required.
