We are looking for a full-time pharmacist to join Humber Teaching NHS Foundation Trust.
The post holder will be a pharmacist, supporting and working alongside a team of pharmacists in general practice. They will be supported by a Specialist Clinical Pharmacist who will develop, manage, and mentor them.
The post holder will work as part of a multi-disciplinary team in a patient-facing role across the trust.
Responsibilities include providing primary support to healthcare professionals regarding prescription and medication queries, supporting the repeat prescription system, handling acute prescription requests, and conducting medicines reconciliation during transfer of care for safer prescribing. They will also provide clinical leadership on medicines optimisation and quality improvement.
This role is pivotal in improving the quality of care and operational efficiency, requiring motivation and passion to deliver excellent service. It is a rotational position with a focus on training and development, with increasing responsibilities as experience grows.
The post may offer the opportunity to undertake a Clinical Pharmacy Diploma or an equivalent post-registration training program in line with NHS England recommendations. Progression to a band 7 post is possible upon successful completion of training and evidence portfolio aligned with the Royal Pharmaceutical Society framework.
The role is a Band 6-7 transitional position, with progression to band 7 contingent upon meeting specified training requirements.
You will be expected to adhere to the GPhC standards for pharmacy professionals.
