Offer description

An excellent opportunity has arisen for a highly motivated, enthusiastic and caring person to joinour Peripatetic Community Support Worker Team, supporting District Nursing teams throughout the Dudley Borough. You will be part of a team providing high quality nursing care to patients in the community, driver essential. Minimum training requirements is a qualification in NVQ Level 3 Health and Social Care.

Previous experience as a Health Care Assistant in Acute or Community Nursing setting essential. You will have good communication skills and be able to work independently and as well as part of a team.


Main duties of the job

The post-holder is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will carry out all relevant forms of care and will take responsibility for caseload holders in their absence.

Person Specification


Qualifications and Education

* Proactive in learning and development of self, and show a willingness to retain information.
* Experience as a Health Care Assistant in the acute or community nursing setting
* NVQ Level 3 in Health and Social Care
* English and Maths Qualifications at one of the following academic levels/grades:- O Level - Grade A to C CSE Grade 1 only GCSE Grade A to C GCSE Grade 9 to 4 Functional skills Level 2
* AS/A Level, NVQ Level 3, Vocational Level 3 qualifications, or equivalent relevant knowledge.


Experience

* To show an ability to develop working relationships with other team members and professionals
* Able to comprehend instructions
* Able to undertake training and competencies relevant to the post.
* Experience in NHS or equivalent healthcare or people facing roles.
* Ability to communicate effectively both verbally and in writing.
* To problem solve quickly, accurately and innovatively
* Basic level of IT skills including email, Word and Excel packages.
* Interest in working in community settings.
* Able to accurately assess, monitor and document risk
* Evidence of being able to work as part of a team
* Knowledge / experience in diabetes care.

* The ability to keep all information confidential
* To be able to work independently in the home and provide feedback to other team members
* Able to communicate accurately in both verbal and written forms
