Community Mental Health - Named Worker Role

The South Community Mental Health Service is undergoing an exciting transformation with additional resources to provide ongoing intensive support to the Sheffield population. This includes working towards a joint approach with Primary Care and the Voluntary Sector, reviewing the CMHT Service Criteria, and working in small teams within local communities to ensure high-quality care.

We are recruiting a Band 6 Nurse or Social Worker to the South CMHT. The successful candidate will receive professional and leadership development through supervision with experienced team managers. We offer regular CPD sessions, and candidates are encouraged to share their interests in mental health. Please note, this is a qualified Nursing vacancy.

You should possess excellent communication skills and be able to work independently and as part of a team to assess, plan, and evaluate care. A positive attitude towards meeting service users' needs, motivation, and enthusiasm for learning and sharing are essential.

The role involves working within a duty system operating from 09:00 to 17:00, Monday to Friday, on a rotational basis, supported by other clinical professionals and management.

Core responsibilities include:

1. Acting as a Named Worker
2. Conducting comprehensive assessments of new referrals
3. Managing a varied caseload based on need
4. Participating in a duty rota
5. Supervising junior staff
6. Contributing to service development

Clinical duties include triaging referrals, assessing health and social care needs, managing clinical risk, formulating care plans, delivering evidence-based interventions, reviewing care effectiveness, working within safeguarding processes, collaborating with partners, maintaining accurate records, and acting culturally sensitively.
