Job Title: Care Coordinator Home Care (Full-time)
Job Summary:We're looking for a qualified and experienced Care Coordinator to join our team at Topioneer Care. As a Care Coordinator, you'll play a vital role in managing and monitoring the quality delivery of our Home Care service for elderly and disabled individuals. This is a flexible field and office-based role that requires excellent communication skills, strong organizational abilities, and a passion for delivering high-quality care.
Key Responsibilities:- Monitor staff and client communications- Manage and maintain accurate rotas- Respond to client complaints and queries- Ensure good record-keeping practices- Conduct care assessments, develop care plans, and risk assessments- Register and interview Domiciliary Care Workers- Conduct supervisions and quality assurance checks- Participate in the 'On Call' system and cover evenings and weekends- Complete field days and care emergency assignments as required
Key Skills and Qualifications:- Qualified to a minimum of Level 3 in Health and Social Care- Strong social care experience and ability to manage pressure- Excellent communication and teamworking skills- Experience in rota planning using staff roster software- Knowledge of CQC standards and compliance- Ability to provide excellent customer service

RequirementsProven experience in an office environment or administrative role is preferred Excellent organisational skills with the ability to manage multiple priorities effectively Strong communication skills, both written and verbal, with a compassionate approach towards clients Ability to work independently as well as part of a multidisciplinary team Proficiency in standard office software programmes such as MS Office Suite A genuine interest in supporting individuals through personalised care coordination Flexibility to adapt to changing priorities within a dynamic environment
