About us
We offer a range of services across Berkshire, with a focus on Reading, Wokingham and Newbury. The main base for our Clinical Nurse Specialist Team is at our Theale office near Reading, however we are open to applications from nurses across Berkshire and can discuss flexible working options.
At the Duchess of Kent Hospice in Reading and through our services in Newbury and Wokingham, we look after people living with life-limiting conditions and offer support for families. We provide expert care and support across Berkshire.

About the Role
Sue Ryder invites applications from experienced nurses to join a well-established, friendly team as a Clinical Nurse Specialist in Reading. You will support patients with an End of Life diagnosis to remain in their homes. This role is predominantly a telephone triage role.
The duties include assessing referrals for admission, performing comprehensive patient assessments, planning, implementing, and evaluating care based on healthcare needs, identifying potential palliative care emergencies, and developing action plans to mitigate distress. You will also develop, document, and communicate these plans collaboratively with other professionals, patients, and families.
Additional responsibilities include:
* Using advanced assessment skills to identify reversible symptoms, develop care plans, and implement them
* Providing holistic, specialist care tailored to patient needs, wishes, and culture, including clinical assessments, care planning, progress monitoring, requesting investigations, and acting on results
* Establishing and maintaining effective communication with patients, carers, relatives, and professionals across health and social services, often of a complex and contentious nature, using verbal and written skills to break down barriers
About you
We seek a registered nurse with experience as a community palliative care nurse or a nurse with palliative care experience and evidence of ongoing professional development in this specialty. Excellent communication skills and qualifications in advanced clinical assessment, mentorship, and non-medical prescribing are required or should be pursued through training.
