Senior Social Worker
Warwickshire County Council is looking for an experienced Senior Social Worker to join our Hospital Social Care team. You will provide a high‑quality social work service for older adults and their families and unpaid carers across three hospital sites in Warwickshire and Coventry.
About the role
As a member of the Hospital Social Care team you will:
* Manage a caseload of older adults and carers, travelling throughout North and South Warwickshire.
* Undertake and record robust assessments and care‑and‑support planning.
* Communicate with customers, communities, health partners and agencies, often working independently in pressured situations.
* Participate in multidisciplinary meetings and team meetings as directed by the Team Manager.
Key Requirements
* Professional Social Work qualification and registration with the Health and Care Professions Council.
* Availability to work 5 days over 7 days, including at least one weekend per month (4‑week rota). Driver’s licence essential for post‑discharge reviews in care homes.
* Experience working closely with a multidisciplinary health and social care team.
* Dynamic approach, ability to make difficult decisions and meet tight deadlines.
* Adult social care experience and good IT skills, including familiarity with the Mosaic IT system.
* Willingness to learn within a fast‑paced environment; hospital social care experience desirable.

Additional Information
