£31,537 – £33,699 per annum.

South Mental Health Social Work Team.
Role Overview
The Mental Health South team supports individuals aged 18+ with mental health conditions, manages community risk, and facilitates discharge plans from out-of-county and rehab wards. They work with partner agencies and support transitions from child to adult services.
Responsibilities
* Provide person-centred social care for adults with mental health conditions.
* Collaborate with health, commissioning, safeguarding and partner agencies to create care plans.
* Manage risk and support discharge planning from out-of-county and rehab wards.
* Support transitions from child to adult services when required.
* Engage in reflective practice sessions and professional development opportunities.
Qualifications & Experience
* Relevant social work qualification or experience in mental health settings.
* Experience in risk management and discharge planning is desirable.
