Job Description - Level 3 Support Coordinator (25912)
Location: Northumberland - Blyth and surrounding areas
Type: Permanent, full-time (37.5 hours per week), Night shifts
Salary: from £25,838 to £26,645 per annum depending on skills and experience, plus benefits including Health Cash Plan
About the Role:
We are seeking a dedicated night worker with alertness and energy to support our customers with complex mental health, learning disabilities, and dual diagnosis. Your responsibilities will include creating support plans, assisting with daily living activities, conducting risk assessments, and collaborating with colleagues.
Key Responsibilities:
* Develop and implement support plans with customers
* Assist with daily activities such as meals, shopping, and budgeting
* Support customers in maintaining tenancy and moving to independent living
* Perform risk assessments, support planning, and review progress
* Engage in regular team meetings and support colleagues
Candidate Requirements:
* Passion for supporting customers to live their best lives
* Experience in creating person-centered support plans
* Experience in coordinating customer referrals
* Ability to work independently, remain calm under pressure, and be resilient
* Own vehicle insured for business purposes (mileage paid)
Additional Information:
* Flexibility in working hours to meet customer needs
* Night shift pattern: 8pm to 8am, 3 days on, 3 days off
* Regular daytime meetings and training sessions
* Enhanced DBS check required; paid for by us
