What you will do:
Respond to requests for Care Act assessments, Mental Capacity assessments, support planning, and reviews.
Handle duty work including hospital discharge planning, ensuring a smooth transition for patients returning to their communities.
Collaborate with health professionals and local services to manage risks and prevent unnecessary hospital admissions.
Participate in supervisions, both individually with the team manager and in group settings, to enhance your practice.
Facilitate informal support and sharing of best practices through daily team huddles and interactions with senior social workers.
