JOB PURPOSE
To provide a range of support to young adult carers aged 14 to 25 to support their transition to adulthood, working in partnership with the adult service provider to form a discreet Transitions Service. This will include:
· Transition Assessments to identify needs in relation to Transitions and the formulation of a goal-based action plan to support any skills, practical support and information, advice and guidance necessary to support successful transition.
· To support Young Adult Carers to complete a Transitions Passport which will contain support requirements for other agencies to better support Young Adult Carers in a variety of settings and a practical action plan to address barriers to progression.
· One to one case load – to offer enhanced information, advice and support to Young Adult Carers who may face additional barriers to successful transitions because of their circumstances.
· Group work with young adult carers, including offering a range of social or skills-based sessions to reduce social isolation and enable them to develop the skills to thrive in their caring role and aide positive progression.
· To actively contribute to the successful development of the service and engage with young adult carers in the development of the service offer
