We are seeking a motivated and compassionate nurse to join our multidisciplinary team. This role is central to delivering high-quality, patient-centred care for individuals living with pulmonary hypertension (PH). This is an excellent opportunity for a registered nurse to develop specialist skills.
You will provide care to patients undergoing investigation, treatment and long-term management of pulmonary hypertension, and their carers. This will be mainly in the daycase ward setting or our Monday morning outpatient clinics and will be supported by the PH CNS team.
You will also be responsible for entering complete and accurate clinical information into the Imperial PH database. This feeds directly into the NHS National Pulmonary Hypertension Audit which measures the quality of care provided to people referred to all PH services. You will liaise with the Lead Clinician and Nurse Consultant to provide regular reports of activity and outcomes.

Main duties of the job
The Band 5 nurse will join the multidisciplinary PH team to provide consistent high-quality care for patients attending daycase and outpatient appointments undergoing investigation, assessments and starting on specialist PH drug therapies.
They will:
1. Work collaboratively with the multidisciplinary team to coordinate patient care and maintain accurate clinical documentation and patient records.
2. Ensure the PH database is kept up to date with the input of accurate clinical information.
3. Contribute to audit, quality improvement, and service development activities and suggest any change to improve standards.
4. Ensure clinical equipment and environments are safe and well maintained.
5. Provide education and support to patients, carers, and families including training for patients on complex treatments, including injected, nebulised and intravenous therapies.
6. Engage with key stakeholders, including the PHA-UK Patient.
7. Association, PH Professionals Forum and the Pulmonary Hypertension National Audit Group.
8. Communicate effectively at all times and maintain a harmonious working environment.
9. Maintain professional standards in line with the NMC Code of Conduct.
10. Actively engage in appraisal, learning, and professional development.
11. Work in accordance with Trust policies and procedures.
