Practice Nurse - Whitchurch Medical Centre
The Lead Practice Nurse will provide clinical leadership and management to the nursing team, ensuring the delivery of safe, high-quality, evidence-based care to our patient population. The post holder will manage their own clinical workload, lead on chronic disease management, and take a proactive role in practice development, clinical governance, and quality improvement initiatives.
Main duties of the job
Provide day-to-day clinical leadership and managementsupport to the nursing team.
Participate in staff appraisals, supervision, mentoring, andprofessional development.
Ensure the nursing team adheres to practice policies,protocols, and standards.
Contribute to service planning and development in line withpractice and local health priorities.
Lead on infection prevention and control within thepractice.
Clinical Duties
Deliver high standards of care in chronic disease management(e.g. diabetes, asthma, COPD, hypertension).
Undertake general treatment room duties including woundcare, immunisations, cervical cytology, family planning advice, and travelhealth.
Carry out health assessments and screening activities suchas new patient checks, NHS Health Checks, and over-75 reviews.
Provide support and education to patients on self-managementand lifestyle advice.
Participate in the implementation of national screening andimmunisation programmes

The Practice is situated in Whitchurch and it benefits from a range of shops and restaurants locally. Whitchurch Medical Centres team comprises of 5 GP Partners, and 5 GP assistants, 3 Practice Nurses, 3 Healthcare Assistants and is supported by a highly effective and friendly admin and reception team. We are proud to be a research ready, teaching practice for medical and nursing students as well as a training practice.

Engage in continuing professional development (CPD) andmaintain up-to-date clinical skills and knowledge.
Participate in regular audits and quality improvementprojects.
Clinical Governance
Ensure accurate and timely documentation of all patientencounters using EMIS.
Work within the scope of practice and follow relevantclinical guidelines and protocols.
Understand and implement Patient Group Directions (PGDs) andassociated policies.
Take a lead role in safeguarding procedures andmulti-disciplinary team discussions.

The post holder will be required to comply with all healthand safety policies, data protection regulations, and the requirements of theNursing and Midwifery Council (NMC). Person Specification
Qualifications
* Registered Nurse (Nursing and Midwifery Council)
* Mentor or teaching qualification
* Postgraduate diploma or degree (Chronic Disease Management)
* Nurse Prescriber
* Completed or working towards formal infection prevention and control training
Additional Criteria
* Skill
* Wound care / removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Requesting pathology tests and processing the results, advising patients accordingly
* Understanding the importance of evidence-based practice
