Overview
Job Summary The post holder is a caring, committed, and experienced nurse who, acting within their professional boundaries, will provide high quality care for patients from initial history taking through to clinical assessment, diagnosis, treatment, and evaluation.
They will demonstrate safe and competent clinical decision making, delivering expert nursing care including assessment, diagnostic skills, and prescribing where applicable, within the general practice environment.
Responsibilities
* Diagnosing and Treating Patients with Minor Illness: Undertake triage and assess patients requesting to see a healthcare professional.
* Provide appropriate treatment and or make necessary referrals to members of the primary healthcare team.
* Inform patients of test results and provide appropriate counselling and follow up advice.
* Ensure clinical practice is safe, effective, and within the scope of professional competence, acknowledging limitations.
* Offer advice on general health care, minor ailments and refer to other primary or secondary care services when required.
* Management of Chronic Disease: Contribute to chronic disease management through assessment, planning, monitoring, and reviewing patient progress.
* Support the clinical team in identifying, recalling, and targeting patients for assessment and treatment.
* Work within NHS and practice protocols to deliver effective long term condition management.
* Providing Assessment, Treatment, Screening Services & Health Education: You will provide highquality nursing care across a broad range of services including: Chronic Disease Management, Asthma, COPD, Diabetes Screening & Womens Health, Health screening, Cervical screening (smears/cytology), HRT, Contraception, Depo injections, pill checks, Removal of IUCD/IUS, Pessary care, Immunisations & Injections, Routine immunisations and vaccinations, Childhood immunisations, Travel vaccinations and followup, Routine injections.
* Treatment Room Duties: Wound care and dressings, Removal of sutures and clips, Minor injury/illness assessment, Assisting with minor procedures, Breath tests, Phlebotomy, ECGs, Blood pressure monitoring, Urinalysis.
* Health Promotion & General Practice Support: New patient registration checks, General lifestyle and dietary advice, Supporting patients with self management, Contributing to health education initiatives.
