Overview
We are recruiting for an Integrated Care Team role within The Mid Yorkshire Teaching NHS Trust. This post supports 24-hour nursing care across Wakefield Area and requires flexibility with working patterns to cover shift patterns, weekends and public holidays. The role involves supporting safe hospital discharges, facilitating communication and coordination of integrated services, and acting as a key contact point to assess, plan and evaluate care while delivering safe and effective care for our patients.
We aim to support you to develop in your new role with a comprehensive induction program. An informal chat with staff in the field is encouraged to discuss different roles.
Responsibilities
* Provide care to patients managed by the Integrated Care Team caseload, including palliative care at home, rapid response to stay at home, and rehabilitation after injury or illness.
* Support out-of-hours district nursing visits and work to prevent hospital admission by coordinating care at home with the Virtual Ward MDT.
* Communicate effectively across services including Reablement and other partners.
* Work flexibly to support shift patterns, weekends and public holidays.
* Demonstrate safe, high-quality care and maintain a safe working environment; utilise reflective practice for personal development.
* Manage workloads, staffing levels, and service capacity to meet care service needs; work on own initiative and as part of a team.
* Utilise IT systems, demonstrate time management, negotiation, and assertive communication skills.
* Possess a Full Clean Driving Licence and access to a car for business use.
Qualifications
* GCSE English and Maths or equivalent
* NVQ level III in Health and Social Care or equivalent experience
* Certificate of Higher Education (equivalent to 100 CAT points) or equivalent experience
* Experience of working as part of a team and coordinating carer visits
* Experience in caregiving environments, dealing with the public, and maintaining confidentiality
* Computer literacy; ability to communicate clearly in writing and verbally
* Ability to analyse and evaluate information; ability to monitor and evaluate Health Care Support Workers input
* Ability to prioritise workloads, manage staffing levels, and contribute to service capacity planning
* Ability to work under pressure, adapt to new environments, and use negotiation and assertive skills appropriately
* Willingness to undertake relevant training
* Inter-personal and organisational skills; trustworthy and reliable
