Job Overview
Join our Complex Discharge Support Team at Glan Clwyd Hospital as Team Manager. The role focuses on supporting patients to return home earlier from hospital through complex case coordination.
Applications will only be considered from staff currently employed by Betsi Cadwaladr University Health Board.
Main duties
* Lead and manage the team to ensure seamless and safe discharge planning of complex patients.
* Coordinate and supervise team members to deliver a patient‑centred service.
* Deliver discharge training programme to new starters.
Qualifications
* NMC Registered Nurse.
* Post Graduate Specialist Practitioner degree.
* Evidence of ongoing education and professional development.
* Mentorship and assessment training.
Experience
* Substantial experience as a discharge liaison nurse in acute or community settings.
* Experience working with patients with acute or complex needs.
* Varied clinical experience and multidisciplinary working.
Skills
* Ability to work autonomously.
* Analytical skills and interpretation of clinical signs.
* Organisational skills.
* Ability to assess, plan, implement and evaluate complex care needs.
* Delegation of care delivery.
* Supervise, teach and assess staff.
* Mentorship and preceptorship.
* Collaborative working with multidisciplinary teams and other agencies.
* IT skills.
* Excellent verbal and written communication.
* Recruitment training.
* Cannulation and venepuncture.
Knowledge
* Knowledge of the changing healthcare agenda affecting community practice.
* Knowledge of current research and its application to practice.
* Evidence of application of consent.
* Knowledge of Mental Capacity laws, Deprivation of Liberty and its implications for practice.
* Level 3 POVA.
* Level 2 Child Protection.
* Knowledge of data protection and confidentiality.
* Ability to undertake complex risk assessments.

Other Relevant Requirements
* Ability to travel.
* Flexibility, including lone working.
