Adult Complex Care Team – NHS Grampian
We are seeking an enthusiastic and capable Band 3 HCSW to support young adults with complex, life‑limiting conditions in their own homes following transition from paediatric care.
About The Role
As part of our dedicated team, you will deliver high‑quality, evidence‑based person‑centred care to young adults with complex physical health needs and learning disabilities. Working closely with families, guardians, and a wide range of professionals, you will help create a supportive, inclusive environment that enables individuals to live safely and comfortably at home.
This role requires working autonomously, making safe clinical judgments, and escalating any concerns appropriately. Strong communication skills are essential due to the independent nature of the post. Experience in tracheostomy care and ventilation is highly advantageous.
Key Skills Required
* Relevant experience working as a Healthcare Support Worker
* Ability to work independently and as part of a wider multidisciplinary team
* Strong interpersonal and communication skills
* A valid UK driving licence and the ability to drive various patient vehicles
* Commitment to providing safe, compassionate, person‑centred care
