We are seeking a motivated person to join our ward teams in this exciting role as Care Coordinator, crucial to supporting the processes we have in place to deliver efficient and timely care. The Care Coordinator within an acute care ward will be responsible for coordinating aspects of patient care including liaison with other agencies both internal and external to CWP. The post holder will be the central coordination point forward based multi-disciplinary activity, organising and supporting with ongoing regular meetings to facilitate progress to recovery and discharge with relevant CWP team members, relatives and carers and other external agencies.
