Role Summary
The Acute Liaison Nurse will act as a clinical expert and professional resource to the multi‑professional team, patients with a learning disability, their families and carers across the Trust.
Responsibilities
* The post holder will plan and deliver care in accordance with the Trust’s committment to deliver high quality care .
* The post holder will work in collaboration with mainstream hospital services to enable open and easy access to healthcare services for people with learning disabilities.
* The post holder will work alongside health professionals and senior hospital staff to achieve a positive experience and clinically appropriate outcomes, for patients with a learning disability.
* The post holder will initiate and support person centred approaches to the delivery of care within healthcare services.
* The post holder will be a key player in the implementation of the hospital’s action plan for people with a learning disability, including attendance at the hospital Learning Disability Steering Group.
* The post holder will advocate on behalf of individuals with a learning disability unable to access mainstream health services.
* The post holder will act as a resource for the acute hospital regarding the complex health needs of people with a learning disability.
* The post holder will support the acute patient area, where necessary, in the assessment, planning, implementation and evaluation of care plans for patients who have a learning disability and highly complex needs.
